In an analysis of Latin American health insurance claims, Inspector found that 43.4% of pharmaceutical spend showed detectable anomalies — 20.3% waste, 10.8% generic substitution opportunity, 7.3% clinical mismatch, and 4.6% behavioral fraud risk. All detection rules were validated through structured manual review.
